"Ballad Of John And Yoko" b/w "Old Brown Shoe". 1969 Apple 2531, Strong EXC+ picture sleeve, and EXC 45-rpm record, matching East Coast copy; the sleeve is probably closer to NM- (no splits, pointy corners, razor-top), the record is VG++/EXC on both sides with clean labels; this is a pretty nice copy of a tough piece to find.
